DC Intervention Strategies With The Adults Assessment Report

• The purpose of this assignment is to create an assessment report based on your interviewee’s responses. To gather more detailed information about your interviewee’s life, you will conduct additional interviews. The following are the specific components that should be included in your second assessment interview report.

1. Basic Demographic Information

– Older adult’s name, address, date of birth, race, marital status and etc.

– If necessary, consider using a pseudonym to safeguard the interviewee’s confidentiality.

–Physical Health 

– Psychological Health

–Sexual Functioning (optional) 

– Social Functioning

–ADL and IADL 

– Spiritual Assessment

–Financial Assessment  

– Environmental Safety Issues (optional)

3. Assessment Review and Future Plan

–Assess the existing or potential issues of your clients.

–Please provide a brief overview of the main factors that contribute to and sustain the problem situation.

–Discover a range of programs and services that your clients can access, such as senior center programs, nutrition programs, access services, and in-home services. Additionally, provide guidance on how clients can effectively utilize these resources to receive the support they need.

–To ensure success, consider potential obstacles that may arise at the personal, interpersonal, or system level. For instance, unresolved family conflicts could hinder the use of family support. Detail your strategy for addressing such barriers.
